RESUMO

The aims of this study were to determine aberrant antigens (AA) in MM cells and to evaluate their prognostic value and frequency. Methods : bone marrow plasma cells of an homogeneous cohort of 30 newly diagnosed patients with MM were studied. As normal control, bone marrow plasma cells from 4 healthy marrow do-nors were also evaluated. Surface and cytoplasmic antigens were analysed using multiparametric flow cytometry (FACS calibur cytometer and Cell Quest and Paint-a-Gate softwares BD). Plasma cells were identified by gating CD45-/CD38++/CD138+++ expressing cells and AA were identifyed using a panel of fluorochrome conjugated monoclonal antibodies : CD45, CD38, CD138, CD19, CD56, CD117, CD13, CD33, CD28, CD14, CD10 CD20, CD22 and HLA-DR. Re-sults - All the patients but one (96,6%) showed at least one AA and the most fre-quent ones were CD56+, CD117+, CD33+, CD13+. The expression of lymphoid an-tigens was more frequently found in those cases that expressed a great number of AA. The absence of the adhesion molecule CD56 was associated with high levels of ß2- microglobulin (p =0,009) and ionic calcium (p = 0,03), showing its prognostic value in MM. The CD28+ also showed a tendency of association with high levels of ß2- microglobulin. We also found one case of CD19+ MM (rare phenotype in neo-plastic plasma cells). Conclusions : The myeloma cells show a high frequency of AA and it should be highlighted the importance of CD56, CD117 and CD28 as prognostic markers, as well as the utility of FC for the study of minimal residual disease in MM patients by detection of the AA, through multiparametric flow cytometry.
