Evaluation of sensibility, upper extremity motor function and functional performance in children with cerebral palsy / Avaliação da sensibilidade, função motora de membros superiores e desempenho funcional de crianças portadoras de paralisia cerebral

RESUMO

Children with cerebral palsy can show somatosensorial handicap in fine motor skills and, in additional with motor problems, they may influence the development of manual abilities and the capacity to execute living daily skills. The objective of this study was to evaluate sensibility, upper extremity skills and performance in self care of children with spastic diplegia cerebral palsy, and to verify the relation among these variables. We studied two independent groups of 30 children each, age ranging from 7 to 8 years. The cerebral palsy children (GI) were selected at the Associação de Assistência a Criança Deficiente (AACD), and were compared with normal children (GII), all of them enrolled in a public school, both Institutions were in the city of São Paulo. In a sectional study, we used the Two-Point Discrimination Test (D2P), Localization of Tactile Stimuli Test (LET), Sterognosis, Quality of Upper Extremity Skills Test (QUEST) and the Pediatric Evaluation of Disability Inventory (PEDI). To compare the variables, we used the Chi-Square Test (to compare proportion) and the Mann-Whitney Test (to compare continuum measurement). The groups showed statistical significant difference related to age, motor function and functional performance. GI had children 7 months younger than GII, with more left handed children and deficits in all studied areas, except in the LET Test. The Wilcoxon Test was used to compare the relation between right and left sides, showing statistical significant difference in the D2P Test (number of correct answers) in GI and Sterognosis (time) in GII. To verify the relationship among the tests of sensibility, motor function and functional performance, we used the Spearman Rank-Order Correlation Coefficient, showing positive statistical significant correlation between D2Pa and functional performance (PEDI) in GI and between D2Pa and motor function (QUEST) in GII. To verify if the variables of sensibility can predict motor function, we used the Multiple Regression Line (identification of a predict model from motor performance-QUEST), showing that for GII, the sensibility tests used contains a regression line (R2 = 0.5491), emphasizing D2P. To verify if sensibility and motor function can predict functional evaluation, we used the Multiple Regression Line (identification of a predict model to functional performance-PEDI), showing that for GI, the model should be an index of R2 = 0.4569, and that motor function and D2P (time) were the most relevant variables and for GII, R2 = 0.6350, emphasizing D2P (time). This study contributed to a better knowledge of the deficits in sensibility, fine motor skills in children with cerebral palsy, and the correlation among them, contributing to a better rehabilitation model, aiming the development of these children in using their hands in daily living skills.
