Otite media silenciosa em criança : triagem e estudo dos fatores de risco

RESUMO

The effect of persistent efusion in the middle ear during the first years of life and even during childhood, has been evaluated by several researchers. Several of them detected on impairment in development of language and speech, modification of the listening process, learning and behavioral problems. Preventive voicetherapysts and audiologists have attempted to develop trial tecniques in arder to detect children with middle ear effusion, and no evident symptons (silent otitis media). The purpose of this study was to evaluate the importance of the trial in kindergarden and school children with no symptoms, trying to relate risk factors for this specific pathology. 103 children were evaluated aged from 11 months to 7 years, 41,74% were found to have disfunction of the middle ear confirmed by otorhinolaringological exam. Based on these two groups: with and without silent otitis media, the relation between any of the risk factors and the incidence of this pathology was observed. The importance of the imitanciometric trial, became evident as children with middle ear effusion were detected and there were no symptons that would lead the parents or the phisician to suspect of this pathology. The risk factors that were studied showed no significant statistic correlation on the ocurring of this pathology. It is necessary to increase the number of cases in a future study and inclued other possible risk factors to the occurring of this pathology as inheritance; number of members in the family; poor hygiene; inadequad nutrition. We propose periodic trials to detect this pathology and a longitudinal follow-up in kindergarden and school children, in order to determine the presence of persistence effusion in our environment, and thus prevent losses in several aspects of children s development. We propose a better definition of silent otitis in our environment, once the incidence is high, a better divulgation of this pathology in health and educacional center, alerting pediatric physicians and teachers to the matter
