Uma ferramenta computacional para processamento de imagens obtidas por cineangiocardiografia e avaliação da função ventricular esquerda

RESUMO

Cineangiocardiography has been considered the most reliable technique to examine cardiac functions. Although subjective visual interpretation of ventriculography data is the most widely method used, many studies show a high interobsel Verand intraobserver variability. In order to perform a semiautomatic analysis of the left ventricle function, it has been developed a software tool for Unix environment to determine quantitative clinical parameters, allowing to evaluate the cardiac function through the comparison of the results with a normal patient data base. The software developed has two basic modules. The first one has routines for cineangiographic image processing with the purpose of enhancement of the images. The routines developed were: median smoothing, multiple thresholding, histogram normalization and image subtraction. Additionally, it has been implemented routines for edge detection to assist the segmentationof the left ventricle. The second module provides tools for definition of the ventricular silhouettes and for determination of the following clinical parameters: left ventricle regional wall motion indexes, ejection fraction, longitudinal shortening fraction and apical obliteration fraction. For the left ventricle regional wall motion analysis, it has been proposed a method that considers the apex obliteration observed during systole. The analysis is performed in both transversal and longitudinal directions according to a reference system based in symmetric reference points of the final diastolic silhouette. The correction of the apex obliteration in the systolic silhouette seems to be necessary specially for hypertrofic patients, otherwise it could lead to a wrong interpretationof the apex region motion. In the evaluationof the ejection fraction,the ventricle volumes were obtained by the sum of the partial volumes of ventricle sections delimited by the symmetric reference points of their respective silhouettes. The longitudinal shortening fraction and the apical obliteration fraction were determined nom the diastolic and systolicsilhouettemean lines.
